Configurable schedule fields

The following fields on the 'make schedule' are configurable:

1. 'Schedule type' label is configurable.  For some clubs it says 'Destination'.  Any text of a similar length can be configured in the table.

2. The list of schedule types or destination options usually has values, such as Local, Training, Cross Country, but these values are also configurable.  And, the placeholder text in the field to the right of that can be specified for each of the list values.  And, there can be an associated tooltip for each of the values, allowing longer instructions.  (Tooltip not available in the upcoming app.)

3. 'custom field 1' and 'custom field 2' are 2 extra text fields.  By default these fields are hidden unless a label is configured.

4. 'Comment with configurable placeholder text' is a placeholder for the comment field and can have any text with instructions on what you want to be entered.
